Overview
This risk assessment template provides a systematic approach to identifying and evaluating workplace hazards. It aligns with ISO 45001:2018 clause 6.1.2 (Hazard identification and assessment of risks and opportunities) and supports the risk-based thinking requirements of ISO 14001:2015.
Template Structure
The template consists of five interconnected worksheets:
1. Hazard Register
The primary worksheet for documenting identified hazards. Each entry captures the activity or task being assessed, the specific hazard identified, potential consequences, and affected personnel or environmental aspects.
2. Risk Matrix
A configurable 5x5 risk matrix that automatically calculates risk ratings based on likelihood and severity inputs. The matrix follows the standard risk calculation methodology where Risk = Likelihood × Severity.
3. Control Measures
Documents existing controls and proposed additional controls following the hierarchy of control: elimination, substitution, engineering controls, administrative controls, and personal protective equipment.
4. Action Tracking
A dedicated worksheet for tracking risk reduction actions, including responsible persons, target dates, completion status, and effectiveness verification.
5. Summary Dashboard
Provides visual analytics including risk distribution charts, control effectiveness metrics, and trend analysis for management review.
How to Use
Begin by identifying all activities within the scope of assessment. For each activity, systematically identify hazards using techniques such as workplace inspections, task analysis, incident history review, and worker consultation.
Rate each risk using the provided matrix, document existing controls, and determine if the residual risk is acceptable. Where risks remain unacceptable, identify additional controls and assign actions with clear accountability.
