ISO 9001:2015
Supplier Evaluation & Re-evaluation Form
A structured form for assessing new suppliers and monitoring the performance of existing ones.
Overview
Clause 8.4 requires you to evaluate, select, monitor performance, and re-evaluate external providers. This form helps you document this process objectively.
Sections
1. Initial Evaluation (New Suppliers)
- Criteria: Quality certifications (ISO 9001), financial stability, capacity, references, price.
- Decision: Approved, Conditionally Approved, or Rejected.
2. Periodic Re-evaluation (Existing Suppliers)
- Performance Data: On-time delivery rate, quality rejection rate, responsiveness to issues.
- Scoring: A weighted score to rank suppliers (e.g., A, B, C).
- Action: Continue, Probation, or Disqualify.
Best Practice
Don’t treat all suppliers the same. Focus your evaluation efforts on “critical” suppliers—those whose failure would significantly impact your product or customer.
