You must provide PPE where risks cannot be controlled by other means. You also need to prove you provided it.
Features
- Employee Name: Who received it?
- Item Description: Safety boots, helmet, glasses, etc.
- Date Issued: When did they get it?
- Signature: Employee signs to acknowledge receipt and training.
- Replacement Date: When is it due for replacement?
Why Sign?
The signature is critical. It confirms the employee received the item and (crucially) understands how to use it.
