Audit Finding Form (NCR)
A template for documenting a single nonconformity or opportunity for improvement.
Overview
Use one form per finding.
Sections
- Finding Details: Audit ID, Date, Auditor.
- Type: Major NC, Minor NC, OFI.
- Description: The PLOR statement (Problem, Location, Evidence, Requirement).
- Auditee Acknowledgment: Signature confirming they understand the finding (not necessarily agreeing, but understanding).
- Root Cause Analysis: Filled by auditee.
- Correction: Immediate fix.
- Corrective Action: Long-term fix.
- Verification: Auditor signs off that the action was effective.
